I acknowledge that I am requesting a supply of four (4) aluminum tube samples, made of 99.7% purity aluminum, provided blank, empty, and open with the specified enclosures or caps from Pack-Tubes free of charge. I understand that these samples are intended solely for conducting our internal stability and compatibility tests related to any products or formulas that will come into direct contact with the supplied aluminum tubes.

I acknowledge that if additional samples are required, I agree to pay the US $240.00 fee to PACK-TUBES for up to thirty (30) additional tube samples based on the specified filling capacity and measurements, excluding shipping and handling. I authorize PACK-TUBES Studio to charge our account for this special delivery request. NOTE you must pay via credit card, paypal, ACH, etc
